What's so frustrating about the "Johnny" ADR is there's no reason for it. They could have called him Sam or George and a new audience would have thought it just as random a name as Johnny. It would have no bearing to the story but not undo what was already there. Or just cut that whole bit out in the edit, since it's irrelevant to the scene anyway. Sometimes I am mystified by Hollywood editors. It really seems like they went out of there way to make the fans NOT think it was Kirk's brother. I suspect maybe one of them (Lindelof?) thought Kirk as a brooding loner with no family was better and wanted to kill off any vestiges of past continuity.

It's been awhile since I watched ST09. No real interest after the latter 2 movies. I agree it was pretty stupid not to make that kid Sam.
But now that the prime universe is indeed separate from the Kelvin, it actually can be chalked up to random differences, not unlike the mirror universe. Ultimately, Chris Pine isn't the "real" Kirk.
